A. The plan will pay benefits for an accidental injury in most circumstance. For the most part, excluded injuries are standard things like results of an act of war or injuries covered by Workers Comp plans. The exact details can be found in the certificate. There are important exclusions you need to be aware of before you join WBA. These features vary slightly from state to state.
2. Intentionally self-inflicted Injury.
3. Suicide or attempted suicide, whether sane or insane.
4. War or act of war, whether declared or not.
5. Injury sustained while on active duty as a member of the armed forces (except Reserve National Guard Service).
6. Injury sustained while on any aircraft except a Civil or Public Aircraft, or Military Transport Aircraft.
7. Injury sustained while on any aircraft: a) as a pilot, crewmember or student pilot; b) as a flight instructor or examiner; c) if it is owned, operated or leased by or on behalf of the Policyholder; d) being used for tests, experimental purposes, stunt flying, racing or endurance tests.
8. Injury sustained while taking drugs, including but not limited to sedatives, narcotics, barbiturates, amphetamines, or hallucinogens, unless as prescribed by or administered by a Physician.
9. Injury sustained while riding or driving in a scheduled race or testing any Motor Vehicle on tracks, speedways or proving grounds.
10. Injury sustained while participating in or practicing for any professional, semi-professional or intercollegiate sports activity.
11. Injury sustained by insured or dependent over age 18 while participating in or practicing for any martial art, mixed martial art or similar organized fight sports activity including but not limited to boxing, wrestling, karate, judo, "ultimate" fighting, kick boxing or similar disciplines.
12. Injury sustained while driving or riding on motorized wheeled vehicles during off-road use including but not limited to all-terrain vehicles (ATV’s).
13. Injury sustained where a Covered Person is the operator and does not possess a current and valid motor vehicle operator’s license, except in a Driver’s Education Program.
14. Injury sustained flying in an experimental aircraft, ultra light, hang gliding, parachuting or bungee-cord jumping.
15. Injury sustained handling, storing or transporting explosives or fireworks.
16. Injury sustained scaling up cliffs or mountain walls or icebergs.
17. Injury sustained while spelunking (exploring caves).
18. Injury sustained while participating in or practicing for any rodeo event.
19. Injuries incurred from repetitive motion injuries, bursitis, strains and tendonitis not related to a specific Injury.
20. Injury sustained while committing or attempting to commit a felony.
21. Injury sustained while Intoxicated.
22. Injury sustained while driving while Intoxicated.
23. Expenses payable by any automobile insurance policy without regard to fault (This exclusion does not apply in any state where prohibited).
24. Covered medical expenses for which the Covered Person would not be responsible for in the absence of the Policy.
A. Organized or team sports are covered unless it involves Professional, Semi-Pro or College level athletics.
A. If you join as a single, you are the only person eligible for benefits. If you choose the family membership, you, your spouse and your children under age 26 who are dependent on you for support and maintenance, or, your children who are dependent on you for support and maintenance because they are incapable of self-sustaining employment by reason of mental or physical handicap. For exact details and definitions, please review the certificate.
A. The WBA plan will pay up to $5000, $7500 or $10,000 (subject to $100 deductible) per covered person, per accident. In the unfortunate case of multiple accidents during the coverage period, they are covered as separate events with a separate benefit, subject to a new deductible. That means $5000, $7500 or $10,000, per person, per accident!
A. Yes. All of the normal provisions apply.
A. The earliest effective date you can get is the 1st of next month.
A. Claims are simple. After your injury, call the toll free number listed in your membership kit. The claims staff will take your information, create a file for you and get a partially prepared claim form out to you in the mail. Remember to save ALL of your bills and receipts. The claims staff will need to see copies of actual bills and invoices.
A. No. Since the WBA plan covers only accidental injury, we strongly advise that you have other coverage, but it is not required.
A. The accident plan will pay for your actual out of pocket expenses, up to $5000, $7500 or $10,000, subject to a $100 deductible. If your health insurance has a deductible and/or co-insurance provisions, you normally would end up owing those amounts out of your pocket. WBA is intended cover the out of pocket portion of the medical bills after other carriers pay first.
A. Yes. Always file a claim with your primary plan first, and then file with WBA IMMEDIATELY after filing with your primary plan. WBA will need to see the documentation of how your health insurance company handles the claim. WBA will pay benefits AFTER your primary plan has completed their benefit responsibilities.
A. Yes. You must file you initial claim with the carrier within 30 days of the injury, or as soon thereafter as is reasonably possible.
A. Generally, claims payments are made directly to the medical providers who are owed money. However, if you have receipts showing that you already paid out of your pocket, the claims payments will be made to you.
